“The intent of the patient-centered specialty practice is to ensure that those high-referred specialty groups and clinicians are making sure that they’re actually appropriately responding to referrals in a timely fashion, ensuring that there’s the appropriate medication management,” says Ms Cooke. “It’s important to understand that care coordination is critical in order to be able to really deliver patient-centered care that addresses the wide variety of care needs of their shared patient population.”
