Based on his personal experiences in the hospital setting, Patrick White reviews the factors that organizations should consider when measuring the success of their transitional care programs.

Mr White explains that, rather than focusing only on quality and short-term costs, organizations need to also assess the long-term costs of their programs to better determine overall cost-effectiveness.

Mr White discusses how some programs are beginning to consider improved technology options and higher-cost tools that, in the long-run, will reduce costs. However, he explains that developing a standardized model to measure what is truly effective will take several years, as there are so many disease cases to explore.
