How does a combination of technology and successful case management help achieve value-based care?
You need both. You truly need both. Technology is good, but it’s a tool in our tool box. With case managers, we have a big tool box that has lots of tools, and we don’t just rely on one tool to drive an outcome if we’re going to call a physician if some is having an exacerbation of a chronic disease. So, what the technology does for us is it opens our eyes. It says, “Hey, the patient might be getting in trouble, let’s pick up the phone, let’s give them a call, let’s do a home visit.” We need to move to the next level to see really what’s wrong with that patient. Having them really work together is what drives the success, not working separately.
