As with UPMC Health Plan’s Freedom House 2.0 and Pathways to Work program, whose foundation is infused with a holistic approach to community support—please check out our second episodes in this series, “Overcoming Employment Barriers for Lasting Social Impact: Freedom House 2.0 and Pathways to Work,”—Clarke’s background as a health activist and wellness advocate guided her to approaching community health holistically through spiritual and community engagement with faith-based organizations and Medicare-enrolled members. This includes efforts, under her program leadership, that address social determinants of health and cultural barriers by stressing the importance of physical, mental, and personal wellness.
