In this final portion of the interview, Dr. Graham addresses Geisinger’s medical home model and how it works with the health system’s telemonitoring program.
The study was built with patients in the medical home to see the effect of an added telemonitoring program. Dr. Graham notes the importance of making sure these quality improvement initiatives are worth the money being spent on them, and that they improve patient lives.
