Amy Berman, BS, RN, says that all too often, people end up in the hospital because they don't know where else to go for treatment.

Amy Berman, BS, RN, says that all too often, people end up in the hospital because they don’t know where else to go for treatment.

“The opportunity in community-based support is that we can prevent people from needlessly going back to the hospital when they have serious illness including cancer,” says Ms Berman.

She adds that palliative care is also patient-centered care.

“Palliative care is person-centered to the core,” suggests Ms Berman. “Person-centered care really is about addressing the total need of the patient, not just a clinical condition, but where that patient wants to go in terms of their health.”
